IMMUNIZATIONS

When students enter high school, parents are required to present a certificate of immunization from a licensed practicing physician or the county health department at the time of registration.

Newly admitted students will be granted 30 days in order to catch up with missing immunizations. If no documentation is received within 30 days from enrollment, the student will be excluded from school.

ALL STUDENTS ARE REQUIRED TO HAVE THE FOLLOWING:

DPT

Diphtheria, Pertussis (whooping cough), and Tetanus: 4 doses are required; an additional Tdap must be given at 11 years of age

IPV/OPV

Polio: 4 doses must be given, with the last dose after 4 years of age

HEPATITIS B

3 doses are required 

MMR

Measles, Mumps, and Rubella (German measles): 2 doses are required. The first dose must be given after 1 year of age, and the 2nd dose must be given after 4 years of age.

VARICELLA

Chickenpox: 1 dose is required after 1 year of age. A second is recommended between 4-6 years old. It is usually given with the MMR.

MENINGITIS 

1 dose is required after 11 years of age. A booster is recommended, but not required at 16 years of age.
