Dear Incoming 7th and 8th Grade Parents/Guardians:

Welcome to the beginning of what will be another exciting school year. I am looking forward to working with you to ensure that your children maintain optimal health. Please remember the following:

  • Physical Exams and Mandatory Immunizations:

  • Physical Exams and Immunization records are required for all new transfer (students from out of district, state or country) 7th and 8th-grade students into the district. In-district students are highly encouraged to get physicals.

  • There are two (2) mandatory 6th-grade vaccines (Tdap and Meningococcal) that I must have documentation of. Please make sure that you send in documentation that the vaccines were given. The CDC and the Englewood Department of Health require that students be compliant with all vaccines. 

  • Physical forms can be found on my Health Office website:

    • Go to www.epsd.org ---> click on Sites and select Janis E. Dismus Middle School (JDMS) ---> go to Menu and select Health Services by clicking on the plus sign (+) or on the words Health Office---> clicking the plus (+) sign opens a drop-down menu ---> select Medical Forms. If you clicked on the words Health Office, you will be directed to my Home Page. The Medical Forms link is located on the left side ---> select either the Universal Health Record or Sports Physical depending on the physical you require.

  • 7th grade guardians—if you do not wish to have your child screened for scoliosis, you have two options.

    §  Either have your child's doctor address it on their physical or

    §  Complete and hand in the OPT-OUT FORM on my website.

  • «Medications in School:

  • Students who will need to take medications in school or need to have medications held in the Health Office (inhalers, nebulizer treatments, diabetes care, Epi-Pen) must have the proper orders from their physician and a parent/guardian signature.

  • Each student must have their own form(s) and each medication must be on a separate form.

  • Medication, Asthma, and Severe allergy/ food allergy forms can be found on my Health Office website:

    • Log into JDMS as instructed above---> go to Menu and select Health Services by clicking on the plus (+) sign or on the words Health Office as above ---> select Physician and Parent/Guardian Medication Authorization Form, Asthma Action Plan, Severe Food Allergy Plan depending on which form(s) you require.

  • «Emergency Contact Form and Health Assessment Form:

  • Please complete an Emergency Contact Form and a brief Health Assessment form for your student(s).

  • Each student must have their own Emergency Contact Form and Health Assessment Form.

  • It is imperative that I have your contact information so that I can reach you for illness or injury issues.

  • Emergency Contact and the Health Assessment Forms can be found on my Health Office website:

    • Log into JDMS as instructed above ---> go to Menu and select Health Services by clicking on the plus (+) sign or on the words Health Office as above ---> select Emergency Contact Form and the Health Assessment Form.
