Universal Health Record or Sports Physicals

The Universal Health Record and Sports Physical are both good for one year. If the student participates in sports, he/she must complete an Update form for each season (Fall and Spring) as per school athletic regulations.

The student’s parent/guardian is responsible for completing this form at the time of new student registration and whenever any medical changes occur. This form will be reviewed by the School Nurse and all student health alerts will be identified and if necessary, clarified with the parent/guardian and/or licensed health care provider. Please make sure to update the Nurse of any new allergies (food of medicine) or medical conditions that your child(ren) has/have. It is important for the Nurse to maintain accurate medical records.

Health Screening Guidelines

Screening programs are designed to provide health appraisals for the identification and management of actual or potential health problems, which include but are not limited to: nursing assessments, vision, hearing, scoliosis, and growth and development. The purpose of the screenings are not to diagnose, but to identify children who may be in need of further evaluation to determine if treatment is necessary. In addition, information may also be required for Early Intervention, Child Study and Intervention and Referral Services.

School Nurses are mandated by the State to do the following screenings if no new annual physical (signed and dated by a physician) is received from the parent/guardian prior to screening date. The majority of the screenings are done between October-December.

6th Grade- Height / Weight / Blood Pressure / Vision

7th Grade- Height / Weight / Blood Pressure / Hearing / Scoliosis (*)

8th Grade- Height / Weight / Blood Pressure / Vision 

Scoliosis screening is done biennially for students between the ages of 10 and 18. If a parent/guardian should refuse scoliosis screening, the parent/guardian must sign and return the opt-out of scoliosis screening form.

Referral forms will be sent to the parent/guardian as needed.
