The Universal Health Record is good for one year. It is important for the School Nurse to have up to date physicals and immunizations. If you child has an annual physical scheduled, please submit the following forms to the School Nurse.

The New Jersey Department of Health requires that all children attending school MUST have a complete record of immunizations on file.Newly admitted students will be granted 30 days in order to catch up with missing immunizations. If documentation is not received within 30 days from enrollment, the student will be excluded from school.

The table below explains the minimum required vaccines a child needs in order to be considered compliant.

Please check with your pediatrician to make sure that your child’s immunizations are up to date. Any time your child receives a vaccine, please forward documentation to the school nurse, so that it can be updated in our system. Your physician may fax a copy to (201) 871-4751.
